The driver involved in a Monday afternoon crash into Lake Tenkiller has been identified. According to the Oklahoma Highway Patrol, Tara Mize, 40, of Broken Arrow died when her 2004 Chrysler Pacifica left the roadway of Highway 82 and went off a 40-foot bluff into Lake Tenkiller.
Mize was pronounced dead at the scene with head, leg and arm injuries according to the OHP report.
The cause of the crash is still unclear and it is not known if Mize was wearing her seat belt.
